The necessary forms for attendance at Green Blaze, National Youth Leadership Training. These forms are only for those scouts who are registered with council.

You will need to bring this equipment with you to camp. If you have a problem with anything on this list, please check with your Scoutmaster for help. Please note that each Scout is required to have his own tent and will be bunking alone.

Please, fill out the Personal Resource Questionnaire we need us this to assign you to a patrol that best suits you and your skills.  This is a fillable PDF that is to be printed and mailed.

The Personal Health and Medical Record form must be completed and presented on arrival at camp.  Most Scout’s have a medical form that is required for either summer camp or high adventure activities.
